What you can expect
We provide a comprehensive one-on-one examination
and assessment with an expert Physical Therapist. If
necessary we will develop a program designed specifically
for you based on your examination and your personal
health and rehabilitation goals.
For your first session
Please arrive 10 minutes before your scheduled appointment time. There will be some paperwork to complete prior to your evaluation and treatment. You can call or email our office to request your paper work via fax or email prior to your appointment, or simply download the forms below. Please bring your insurance card, photo ID, and the prescription for physical therapy written by your physician or referring provider. Your physician may fax the prescription to our office at (949) 429-2151. And be sure to wear comfortable, loose-fitting clothing and comfortable shoes.
